Currently, I am credentialed with Regence and Kaiser Permanente as a Licensed Independent Clinical Social Worker, and am considered an in-network provider for most Regence and Kaiser Permanente PPO plans. I am an out-of-network provider for other carriers and can provide you with the necessary paperwork to submit to your insurance provider. Please check your insurance plan prior to our first appointment to confirm the details of your coverage.
Payment
I accept cash, checks, and credit cards. Payment is due at each session.
Cancellation Policy
A minimum of a 24-hour notice is required for rescheduling or cancelling an appointment or you will be charged the full fee (these are not covered by insurance).
